Overview: The High School Transition Coordinator plays a vital role in facilitating the successful transition of high school students into various post-secondary pathways, including college, vocational training, or the workforce. This role involves collaborating with students, families, educators, and community partners to provide comprehensive support and resources to ensure students' smooth transition from high school to their chosen next steps.

Key Responsibilities:

  1. Student Guidance and Support:

    • Provide individualized guidance to students regarding post-secondary options, including college applications, vocational training programs, and career exploration.
    • Conduct assessments to identify students' interests, skills, and goals to help them make informed decisions about their future.
    • Assist students in developing academic and career plans that align with their aspirations and strengths.
  2. Resource Coordination:

    • Collaborate with school counselors, teachers, and administrators to develop and implement transition programs and initiatives.
    • Establish partnerships with colleges, trade schools, employers, and community organizations to expand resources and opportunities available to students.
    • Coordinate workshops, seminars, and informational sessions on topics such as financial aid, resume building, interview skills, and college readiness.
  3. Family Engagement:

    • Engage families in the transition process by providing information and resources to support their understanding of post-secondary options and requirements.
    • Conduct family outreach activities, including workshops and one-on-one meetings, to address concerns and provide assistance with navigating the transition process.
    • Foster positive relationships between families and educational institutions to promote student success beyond high school.
  4. Data Management and Evaluation:

    • Maintain accurate records of student progress, including academic performance, post-secondary plans, and participation in transition activities.
    • Utilize data analysis to monitor the effectiveness of transition programs and interventions, making adjustments as needed to meet students' needs.
    • Generate reports and assessments to evaluate the outcomes of transition efforts and identify areas for improvement.
  5. Advocacy and Support Services:

    • Serve as an advocate for students, particularly those facing barriers to post-secondary success, such as financial constraints, and/or academic challenges.
    • Connect students with support services, including tutoring, counseling, mentorship, and community resources, to address their individual needs and promote overall well-being.
    • Collaborate with school staff and external agencies to develop intervention plans and support strategies for at-risk students.

Qualifications:

  • Experience working with high school students in an educational or counseling capacity.
  • Knowledge of post-secondary education options, including college admissions processes, financial aid, and career pathways.
  • Strong interpersonal and communication skills, with the ability to engage effectively with students, families, and community stakeholders.
  • Excellent organizational skills and attention to detail, with the ability to manage multiple tasks and priorities.
  • Familiarity with data management systems and proficiency in computer applications.
  • Commitment to equity and diversity, with a focus on supporting underserved and underrepresented student populations.
  • Ability to work independently and collaboratively in a fast-paced environment.
